Segundo a International Society of Travel Medicine (ISTM), os acidentes e as doen\u00e7as cardiovasculares s\u00e3o as principais causas de morte em viajantes, ultrapassando as doen\u00e7as infeciosas.<\/p><p>Em Portugal, a crescente popularidade de destinos de altitude elevada (Andes, Himalaias, Kilimanjaro), viagens de longa dist\u00e2ncia e viagens de aventura torna estes riscos n\u00e3o infeciosos cada vez mais relevantes na consulta do viajante. A prepara\u00e7\u00e3o adequada e o aconselhamento m\u00e9dico pr\u00e9vio podem prevenir complica\u00e7\u00f5es potencialmente graves.<\/p><h2>Jet Lag: Gest\u00e3o e Estrat\u00e9gias de Adapta\u00e7\u00e3o<\/h2><p>O <b>jet lag<\/b> resulta da dessincroniza\u00e7\u00e3o entre o rel\u00f3gio biol\u00f3gico circadiano e o ciclo luz-escurid\u00e3o do destino, afetando viajantes que atravessem 3 ou mais fusos hor\u00e1rios. Os sintomas incluem fadiga diurna, ins\u00f3nia, dificuldade de concentra\u00e7\u00e3o, irritabilidade, perturba\u00e7\u00f5es gastrointestinais e mal-estar geral. Os sintomas s\u00e3o tipicamente mais graves em viagens para leste (encurtamento do dia) do que para oeste.<\/p><p>A adapta\u00e7\u00e3o ao novo fuso hor\u00e1rio demora geralmente 1 dia por fuso atravessado. Estrat\u00e9gias baseadas em evid\u00eancia incluem: exposi\u00e7\u00e3o \u00e0 luz natural no hor\u00e1rio adequado (manh\u00e3 no destino para viagens para leste, tarde para viagens para oeste), ajuste gradual do hor\u00e1rio de sono nos dias anteriores \u00e0 viagem e manuten\u00e7\u00e3o de hidrata\u00e7\u00e3o adequada durante o voo.<\/p><p>A melatonina 0,5 a 5 mg ao deitar (hor\u00e1rio local do destino) \u00e9 o suplemento com maior evid\u00eancia para facilitar a adapta\u00e7\u00e3o ao <b>jet lag<\/b>, conforme revis\u00e3o Cochrane. A melatonina \u00e9 mais eficaz em viagens para leste e quando atravessados 5 ou mais fusos. O NICE reconhece a melatonina como op\u00e7\u00e3o de curta dura\u00e7\u00e3o para <b>jet lag<\/b>. Os hipn\u00f3ticos (zolpidem, zopiclona) podem ser considerados para ins\u00f3nia de adapta\u00e7\u00e3o em viajantes selecionados, sob prescri\u00e7\u00e3o m\u00e9dica.<\/p><h2>Doen\u00e7a de Altitude: Preven\u00e7\u00e3o e Tratamento<\/h2><p>A doen\u00e7a aguda de montanha (DAM) pode afetar qualquer pessoa que ascenda rapidamente acima de 2500 metros, independentemente da condi\u00e7\u00e3o f\u00edsica. Os sintomas surgem 6 a 12 horas ap\u00f3s a chegada \u00e0 altitude e incluem cefaleia, n\u00e1useas, anorexia, fadiga, tonturas e perturba\u00e7\u00e3o do sono. A incid\u00eancia \u00e9 de 10 a 25 % a 2500 metros e 50 a 85 % acima de 4500 metros.<\/p><p>As complica\u00e7\u00f5es graves \u2014 edema pulmonar de alta altitude (EPAA) e edema cerebral de alta altitude (ECAA) \u2014 s\u00e3o emerg\u00eancias m\u00e9dicas que requerem descida imediata. O EPAA manifesta-se com dispneia em repouso, tosse seca, taquicardia e cianose. O ECAA apresenta-se com cefaleia intensa, confus\u00e3o, ataxia e altera\u00e7\u00e3o do estado de consci\u00eancia. Ambas as condi\u00e7\u00f5es podem ser fatais sem tratamento.<\/p><p>A preven\u00e7\u00e3o da DAM baseia-se na aclimatiza\u00e7\u00e3o progressiva: ascens\u00e3o m\u00e1xima de 300-500 metros\/dia acima dos 2500 metros, com dia de repouso a cada 1000 metros de ganho de altitude. A acetazolamida 125-250 mg duas vezes por dia, iniciada 24 horas antes da ascens\u00e3o, \u00e9 o f\u00e1rmaco preventivo de refer\u00eancia, acelerando a aclimatiza\u00e7\u00e3o. A Wilderness Medical Society e a International Society of Travel Medicine publicam guidelines atualizadas sobre preven\u00e7\u00e3o e tratamento da doen\u00e7a de altitude.<\/p><h2>Sa\u00fade Mental, Acidentes e Outros Riscos N\u00e3o Infeciosos<\/h2><p>Os problemas de sa\u00fade mental em viagem s\u00e3o frequentes mas raramente abordados na consulta do viajante. O stress da viagem, o isolamento, o choque cultural, o consumo excessivo de \u00e1lcool e a descontinua\u00e7\u00e3o inadvertida de medica\u00e7\u00e3o psiqui\u00e1trica podem desencadear ou agravar perturba\u00e7\u00f5es de ansiedade, depress\u00e3o e perturba\u00e7\u00f5es psic\u00f3ticas. A Organiza\u00e7\u00e3o Mundial de Sa\u00fade (OMS) recomenda que viajantes com doen\u00e7as psiqui\u00e1tricas mantenham a medica\u00e7\u00e3o habitual e transportem receitas e relat\u00f3rios m\u00e9dicos em ingl\u00eas.<\/p><p>Os acidentes de via\u00e7\u00e3o s\u00e3o a principal causa de morte em viajantes jovens, particularmente em pa\u00edses de baixo e m\u00e9dio rendimento onde a seguran\u00e7a rodovi\u00e1ria \u00e9 deficiente. O uso de cinto de seguran\u00e7a, capacete em motas e bicicletas, e evitar conduzir \u00e0 noite em estradas desconhecidas s\u00e3o medidas preventivas fundamentais. O afogamento \u00e9 outra causa frequente de morte em viajantes, associado a correntes mar\u00edtimas desconhecidas e consumo de \u00e1lcool.<\/p><p>A exposi\u00e7\u00e3o solar intensa, a desidrata\u00e7\u00e3o e os golpes de calor s\u00e3o riscos significativos em destinos tropicais. A prote\u00e7\u00e3o solar (FPS 50+), hidrata\u00e7\u00e3o frequente, uso de chap\u00e9u e evitar exposi\u00e7\u00e3o nas horas de maior calor (11h-15h) s\u00e3o medidas essenciais.
