Com mais de metade da popula\u00e7\u00e3o a viver com excesso de peso ou obesidade, segundo o Inqu\u00e9rito Nacional de Sa\u00fade (INE) e a DGS, a necessidade de abordagens eficazes e seguras \u00e9 uma prioridade de sa\u00fade p\u00fablica. Contudo, \u00e9 fundamental distinguir entre <b>perda de peso saud\u00e1vel<\/b> e dietas restritivas que podem ser prejudiciais.<\/p>\n<p>A avalia\u00e7\u00e3o da necessidade de <b>perda de peso<\/b> deve ser feita por um profissional de sa\u00fade, tendo em conta o IMC, o per\u00edmetro abdominal e as comorbilidades. A OMS considera excesso de peso um IMC entre 25 e 29,9 e obesidade um IMC igual ou superior a 30. A Sociedade Portuguesa para o Estudo da Obesidade (SPEO) recomenda perda de peso quando existe excesso de peso com comorbilidades ou obesidade, mesmo sem comorbilidades associadas.<\/p>\n<h2><strong>Objetivos Realistas: Quanto e a Que Ritmo<\/strong><\/h2>\n<p>As guidelines da European Association for the Study of Obesity (EASO) e do NICE recomendam uma perda inicial de 5 a 10% do peso, a um ritmo de 0,5 a 1 kg por semana. Esta perda moderada j\u00e1 produz melhorias significativas na resist\u00eancia \u00e0 insulina, tens\u00e3o arterial e perfil lip\u00eddico. A expectativa de resultados r\u00e1pidos \u00e9 irrealista e insustent\u00e1vel.<\/p>\n<p>O foco deve estar na manuten\u00e7\u00e3o a longo prazo, n\u00e3o apenas na perda. Estudos demonstram que a maioria das pessoas que perdem peso com dietas restritivas recuperam-no em 1-5 anos. A mudan\u00e7a sustent\u00e1vel de h\u00e1bitos alimentares e de atividade f\u00edsica \u00e9 o verdadeiro indicador de sucesso.<\/p>\n<h2><strong>Alimenta\u00e7\u00e3o: Dieta Mediterr\u00e2nica como Refer\u00eancia<\/strong><\/h2>\n<p>A base \u00e9 uma alimenta\u00e7\u00e3o equilibrada com d\u00e9fice cal\u00f3rico moderado (500-750 kcal\/dia). A Dieta Mediterr\u00e2nica \u2014 reconhecida pela UNESCO \u2014 \u00e9 recomendada pela APN e pela DGS como padr\u00e3o de refer\u00eancia. Caracteriza-se por legumes, frutas, leguminosas, cereais integrais, azeite e peixe, com consumo moderado de lactic\u00ednios e reduzido de carnes vermelhas e a\u00e7\u00facares.<\/p>\n<p>A orienta\u00e7\u00e3o por nutricionista \u00e9 fortemente recomendada para garantir um plano personalizado e nutricionalmente adequado. As dietas muito restritivas (abaixo de 1200 kcal\/dia) podem causar defici\u00eancias nutricionais, perda de massa muscular e desacelera\u00e7\u00e3o metab\u00f3lica.<\/p>\n<h2><strong>Exerc\u00edcio F\u00edsico: Complemento Indispens\u00e1vel<\/strong><\/h2>\n<p>A OMS recomenda 150-300 minutos de atividade aer\u00f3bica moderada por semana, complementados por exerc\u00edcios de fortalecimento muscular pelo menos 2 vezes por semana. O exerc\u00edcio melhora a composi\u00e7\u00e3o corporal (preserva massa muscular durante a perda de peso), o humor, a qualidade do sono e a sa\u00fade cardiovascular.<\/p>\n<p>A combina\u00e7\u00e3o de exerc\u00edcio aer\u00f3bico (caminhada, nata\u00e7\u00e3o, ciclismo) com treino de for\u00e7a \u00e9 a mais eficaz para a perda de peso e manuten\u00e7\u00e3o. A prescri\u00e7\u00e3o deve ser adaptada \u00e0 condi\u00e7\u00e3o f\u00edsica e prefer\u00eancias do indiv\u00edduo. Come\u00e7ar gradualmente e aumentar progressivamente \u00e9 a estrat\u00e9gia mais segura e sustent\u00e1vel.<\/p>\n<h2><strong>Quando Considerar Tratamento Farmacol\u00f3gico<\/strong><\/h2>\n<p>Em doentes com obesidade (IMC \u226530) ou excesso de peso com comorbilidades que n\u00e3o respondem \u00e0s altera\u00e7\u00f5es do estilo de vida, pode ser considerada farmacoterapia. A liraglutida e a semaglutida (agonistas GLP-1) demonstraram efic\u00e1cia significativa e est\u00e3o aprovadas pela EMA. A semaglutida 2,4 mg produziu perdas m\u00e9dias de 15-17% do peso corporal nos estudos STEP.<\/p>\n<p>A cirurgia bari\u00e1trica \u00e9 indicada para obesidade grave (IMC \u226540 ou \u226535 com comorbilidades) ap\u00f3s insucesso conservador.
